We collect this information so we can route your request to an individual at the specific location requested who will contact you to begin conversations about scheduling an appointment.
Access Your Patient Portal. If your clinic location has a patient portal implemented, you may access your patient portal on the Website by providing your unique username and password. We collect this information to provide you access to this service.
Contact Us. When you use our contact form on the Website we collect your name, phone number, location, and the department you wish to contact. You also have the option to provide us with your email address. We collect this information so we can respond to your inquiry.
